Parental Consent for Treatment Related to Child Custody

To authorize mental health treatment for your child, you must have either sole or joint legal custody of your child.

If you are separated or divorced from the other parent of your child, we require:

  • a copy of the most recent legal documentation establishing custody rights or otherwise demonstrates your right to authorize treatment for your child. 
  • written consent from each parent prior to starting services, in accordance with Mississippi state law regarding parental rights.

We must have this documentation on file prior to starting any services. 

Missed Appointments and Late Cancellations

Scheduled appointments that are cancelled with less than 48 business hours’ notice is considered a “late cancellation.”

A missed appointment is also known as a “no-show,” where the client did not call and did not show up to their scheduled appointment (including telehealth).

Late cancellations and missed appointments (no-shows) are subject to a $175 charge, which cannot be billed to insurance.

All scheduling changes should be handled with our client care team during normal business/administrative hours (Monday-Friday between 8:00am and 5:00pm, excluding weekends and holidays). For example, this means that appointments scheduled for Monday must be cancelled by the previous Thursday.

Our client care team can be reached by calling or texting 228.819.2171 or emailing them at hello@phoenixpllc.com, or sending a message through your Patient Portal. While we see clients during evening and weekend hours, we do not have administrative staff available to promptly handle time-sensitive scheduling changes. Since we are routinely booked out for weeks in advance, appointments cancelled with less than 48-business hours’ notice does not allow the opportunity to fill that time slot with another client who is awaiting services.

Late Arrivals

Please call if you intend to keep your appointment but are running late. We also want you to be safe, so if you think you might not make it to the office on time, we are happy to convert your appointment to telehealth as an option.

If you have not called and have not arrived 10 minutes after your scheduled appointment time, you will be considered a “no show” and must reschedule. You will also be charged the missed appointment fee.

Forensic (Court/Legal) Matters

We do not provide forensic psychological/mental health services involving legal matters and court. We maintain our roles as clinicians (e.g., therapist, counselor), with and you or your child as our client, and not forensic evaluators, which makes the court or legal counsel our client. 

Forensic mental health evaluations require additional specialized training and in-depth knowledge of the law and mental health. It is an ethical violation and a major conflict of interest to put a therapist or counselor, who is treating you or your child, in the evaluator role which can create undue harm.

If any employee of Phoenix is subpoenaed to testify on your (or your child’s) behalf, you are fully responsible for all out-of-pocket costs, which are not covered by insurance.

Fees start at $250 per hour related to preparation, documentation, legal consultation, and travel time (if required).

For depositions and court appearances, fees start at $1000 (billed at a half-day/4-hour minimum). Payment is due, in full, within 48 business hours of our receipt of a subpoena.

If we are given less than 7 calendar days’ notice, a $500 expedited fee will be added in addition to the standard fees above. Payment is due, in full, within 24 hours upon receipt of a subpoena.
